Lead form modification

Right now, we can add fields to the lead form, but we can't change the order. We want to request the site address, but if we request that, it shows up after "message", which feels out of order and odd.

I'd also like to determine whether these forms are optional or required, then have a redirect link once they submit (so we can direct them to our scheduling software).
